LOSS OF COVERAGE

If health coverage will be replacing coverage provided by your former health plan, you will have 31 days after the qualifying life event to make any changes. Benefits will become effective immediately after election.

DO NOW:

• Update your qualified benefit plans.
• Make any needed changes to your qualified benefit plans. Remember, adding your spouse, domestic partner, and/or child’s information does not automatically enroll them in benefits.
• Update your Life Insurance and other supplemental coverage Disability, Accidental Death and Dismemberment, Critical Illness, Fixed Indemnity, Accident coverage).
